Feeding Schedule

Veterinary Care and Liability

1. Veterinary Care: Should the dog become ill, injured, or require medical attention, the Boarding Facility will make reasonable efforts to contact the Owner and/or Emergency Contact. However, if the Owner or Emergency Contact cannot be reached, the Boarding Facility is authorized to seek veterinary care for the dog.

2. Veterinary Costs: The Owner agrees to be fully responsible for any and all veterinary costs incurred during the dog's stay, including but not limited to, examinations, treatments, surgeries, medications, and other related expenses. The Owner will reimburse the Boarding Facility for any such costs upon presentation of a veterinary invoice.

3. Release of Liability: The Owner agrees to release and hold harmless the Boarding Facility, its employees, agents, and representatives from any and all claims, damages, or liabilities arising from or related to the boarding of the dog, including but not limited to injury, illness, or death of the dog. This release of liability includes any claims for negligence, breach of contract, or any other cause of action.

Payment Terms

Boarding Fee PER DAY: $60.00 (drop off & pick up days are days - 2 DAYS)

Payment Due: 50% NON-REFUNDABLE at time of scheduling. 50% paid at pick-up.

Holiday Charges

Holidays recognized: New Year's Day, Martin Luther King, Jr. Day, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.

A holiday is considered the day before, the day of, and the day after.

The holiday charge is $75 per day.

Changes to Original Dates

Changes to the drop off or pick up dates are NOT guaranteed and must be made at least one

week prior to the drop off or pick up dates.

Cancellation Policy

Cancellation must be done 30 days PRIOR to the start of the scheduled drop off date. If cancellation is made 30 days prior to the drop off date, the 50% non-refundable amount is transferable to another stay within the SAME calendar year.

Cancellation WITHIN the 30 day window forfeits the 50% non-refundable amount and is NOT transferable to another stay.

Owner's Representations and Warranties

The Owner represents and warrants that:

1. The dog is in good health and up-to-date on appropriate vaccinations.

2. The information provided in this Agreement is accurate and complete.

Miscellaneous

• This Agreement constitutes the entire agreement between the parties and supersedes any prior agreements or understandings, whether written or oral.

• This Agreement shall be governed by the laws of Colorado.

• Any amendments to this Agreement must be in writing and signed by both parties.

By signing below, the Owner acknowledges and agrees to all terms and conditions of this Agreement.
